SHARED LIVING SERVICES
1. A home owned, leased, or rented by the individual, where care and support are directly provided to/for the individual.
2. The individual is incorporated into a family in a residence owned, leased, or rented by the family.

SHARED LIVING WITH NOBLE CARE
Adults with intellectual disabilities and autistic disorders who opt to experience Shared Living while they receive the necessary support to be successful have proven to thrive in these home environments. Under this model, the client is supported by the consistency of a stable environment. This type of immersive community-focused living situation also encourages social growthand a greater sense of autonomy.
Noble Care’s professionals advocate for this type of housing situation to our clients as it is one of the least restrictive and most cost-effective options for home support services available in Maine.
WHO IS SHARED LIVING FOR?
Any adult individual with intellectual and developmental disabilities and/or autism who prefers to live in a family-type home, who is eligible for MaineCare services under the Home and Community-Based Waiver, and whose Person-Centered Planning (PCP) team has determined this to be an appropriate living option. Eligible clients must coordinate this living situation through Noble Care’s professionals and meet the DHHS criteria in Section 21 or MaineCare Section 29.
Developmental Disability Services does not support multiple people with disabilities living in this model unless the people are family members (ex., brother and sister, husband and wife) or have a long-standing relationship/friendship where they are able to clearly indicate their desire to share in this living model. All exceptions to this protocol must be approved by the OADS Program Administrator of the district where the client resides.
WHAT ARE HOME SERVICES?
Whether you decide to receive care in a private home or an agency-managed apartment setting, our Direct Support Professionals (DSPs) will carefully and expertly coordinate care based on the individual's needs. Each of our services are scheduled throughout the week in pre-determined blocks of time.
Our residential group homes are staffed up to 24-hours a day and designed to support various levels and types of needs. Services support across medical, dietary, and behavioral needs that are carefully identified and fall into three categories:

Noble Care consistently adds to our providers list so that we have ample options to create matches near our office in Maine. These efforts allow us to grow accommodations and program offerings. When a need or new provider is added, we quickly work to reate matches.
WHAT STANDARDS
DO PROVIDERS MEET?
The providers meet the standards set by Office of Aging and Disability Services, in the Department of Health and Human Services:
- All adult household members mus successfully pass background checks as required by the MaineCare Benefits Manual.
- Successfully pass a home safety inspection to ensure the home meets all standards set by the MaineCare Benefits Manual and DHHS-OADS.
- Ensure that their home meets local housing codes.
- Verify he/she has time to provide daily services/supports that meet the needs of the individual.
- Have a high school diploma or GED and a valid Maine driver’s license.
- Become DSP certified
- Attend Reportable events training
Noble Care provides Shared Living Support Services in York County and Cumberland County.
STEPS TO TRANSITION A CLIENT INTO A GROUP HOME
When transitioning a client into a group home, there are several essential steps that the company should take to ensure a smooth and successful process. By following these steps, the company can ensure that the client's transition into a group home is a smooth and successful one.

GROUP HOME ELIGIBILITY REQUIREMENTS
Overview: An individual must be enrolled in and have MaineCare and waiver Section 21 or 29 funding through the Department of Health and Human Services, Office of Aging and Disability Services to be considered for Noble Care’s home support services.
To be eligible for Noble Care’s home support services, an individual must meet the following eligibility requirements:
ENROLLMENT IN MAINECARE AND WAIVER SECTION 21 OR 29 FUNDING: Individuals must be enrolled in MaineCare, which is the state's Medicaid program, and they must have waiver Section 21 or 29 funding through the Office of Aging and Disability Services (OADS).
MEDICAL CONDITION: Individuals with a qualifying medical or disability condition are eligible for Noble Care’s home support services. This could include individuals with chronic health conditions, physical disabilities, or cognitive disorders.
ASSESSMENT: Individuals must have completed a comprehensive assessment process conducted by a qualified professional to determine their eligibility for home support services. This assessment will consider the individual's needs, preferences, and functional abilities.
SERVICE PLAN: Individuals must have a written service plan that outlines the specific services and supports they will receive through Noble Care. This plan will be developed by a team of healthcare professionals, including nurses, social workers, and occupational therapists, and will be tailored to the individual's unique needs.
FINANCIAL ELIGIBILITY: Individuals must meet the financial eligibility requirements of MaineCare and waiver Section 21 or 29 funding. This could include meeting income limits, asset limits, and Medicaid spend-down requirements.
SERVICES: Individuals must be eligible to receive the specific services provided by Noble Care. These may include personal care assistance, homemaking services, medication management, transportation, or respite care.
By meeting these eligibility requirements, individuals can qualify for Noble Care’s home support services. These services are designed to enable individuals to live independently and safely in their own homes, promoting their overall health and well-being.
