CASE MANAGEMENT COORDINATION AND TRANSITION PLANNING: We create a Person-Centered Care plan (PCP) that addresses the client’s specific needs, including coordinating all required services.
LEGAL SUPPORT: We provide assistance with legal issues related to your case, such as navigating the court system, advocating for your rights, and representing your interests.
FINANCIAL GUIDANCE: We offer financial counseling to help you manage expenses, overcome financial obstacles, and develop sustainable solutions.
CASE MANAGEMENT SERVICES WITH NOBLE CARE CAN INCLUDE:
- Completing initial application for services
- Development of your Person-Centered Plan (PCP). This includes goals based on identified strengths and needs, and expressed personal preferences.
- Completing applications and renewals for MaineCare Waiver funding.
- Coordination of a wide range of services, advocating for you, referrals, and continuous monitoring of quality and progress.
- Identifying and obtaining medical equipment and assistive technologies.
- Providing home and community based support that will help you achieve and maintain independence through skill-builiding, self-advocacy, and self-expression.
- Linking you to resources that meet your needs, including employment and/or volunteer opportunities.
